Registration Confirmation Page & Consent Forms

We would like to thank you for registering and participating in our SHE Forum / SHE Events.

But you’re not done yet. If you are a parent bringing students to the event, you must download both thes media and medical release forms below, print, fill them out and bring them with you on the event day. This will help with the sign-in process at the beginning of the event. These forms are standard procedure.

Remember, these forms are required for each individual student (minor) that come with you so you may need to print and fill multiple copies.

What do parents think of this event?

Parents all over the Valley of the Sun are singing our praises about the A-Ha moments they had at our workshop. Read a testimonial from a parent that attended this month’s Cave Creek Forum”……

“I remember when my older two daughters would tell me they were “stressed out”…I would think why?  What do you have to be stressed about? – well, after seeing the skit by the young people, I found out.  It literally brought tears to my eyes that I discounted their “stress” – I learned so much – when I came home from the event, I apologized to my 18 year old in person for discounting her feelings and called my 20 year old at college and did the same thing.  The two women who spoke after Dr. Franny’s skit were wonderful – I used their advice and techniques over the weekend with my 18 year old and it went well!  The information on the internet was great and the talk on the drugs was enlightening – a lot different then when I was in high school.  On my survey form, I wrote that this event should be mandatory for all parents – I think it should be part of the middle school/high school open house…I couldn’t do without anything – It also validated some of the things I have in place currently – you know, you can only hear so much how “other parents don’t do this…” until you get broken down.  The event just reaffirmed that I’m doing the right thing – BUT NOW…I will LISTEN and validate my daughters’ feelings instead of questioning them – THANK YOU!”

What about dads at the event?

We strongly encourage fathers and significant others to join you, this is not just for mothers and daughters! Dads, grandfathers, and significant others are welcome to join us ALL DAY.  But if that’s not possible, we also have a shorter segment after lunch, a skit  that begins promptly at 9:15am entitled ” Queenbees and Wannabes”, a powerful and gripping look into a typical day in the life of our tween and teen girls.
Please e-mail us at info@sharinghealthyexperiences.com if dad/etc. will be joining you for the morning skit.  There is no charge however, we need to reserve him a seat! Many moms are leaving our forum saying ” I wish I had brought my husband/boyfriend/father/neighbor……”
